Godparent & Sponsor Information Form
Being asked to serve as a godparent or sponsor is an honor, and we welcome you warmly. Please keep in mind that the Church sets requirements for this role, in keeping with the function you have been asked to serve.
Requirements to be a Godparent or Sponsor:
Be designated by the person to be baptized or confirmed, by the parents or the person who takes their place, or, in their absence, by the pastor or minister, and have the aptitude and intention of fulfilling this function.
Have completed the sixteenth year of age.
Be a Catholic who has been confirmed, has received the Most Holy Eucharist, and leads a life of faith in keeping with the function to be taken on. This includes being in a marriage recognized as valid by the Catholic Church, if married, and regularly attending Mass on Sundays and Holy Days of Obligation.
Not be bound by any canonical penalty legitimately imposed or declared.
Not be the father or mother of the person to be baptized or confirmed, including stepparents and adoptive parents.
If any of these requirements are not met, we will not be able to move forward with your sponsorship at this time. If you are unsure whether you meet a requirement, please contact the Evangelization Office at 480-893-8770 ext.227. We are glad to talk through your situation with you.
Before beginning, please have on hand a Baptism Certificate issued within the last six months, with notations (the record of your confirmation and, if applicable, your marriage, added to your baptismal record). If your Baptism Certificate does not include notations, you will also need a Confirmation Certificate and, if married, a Marriage Certificate.

To continue, you must affirm that you meet every requirement listed above. This form must be completed by the person who has been asked to serve as the godparent or sponsor.
